je souhaite créer une installation automatisée (un gros patch) et un petit jeu.
comment fait-on ?

L'outil utilisé pour créer des versions automatisées est Inno Setup -> https://jrsoftware.org/isdl.php#qsp

Tu veux créé une installation automatique avec Inno setup ? ou juste pouvoir lancer ton jeu automatiquement sans avoir à reconfigurer à chaque lancement ?

Dfend? Pas besoin de passer par Dfend, seul Dosbox est nécessaire. Ensuite Inosetup, il faudrait montrer à quoi ressemble ton fichier source. Celui ou il y a tous les paramètres pour créer ton Setup. Bon après je suis rouillé. J'en ai pas fait depuis pas de 6 ans.
